Our team of chaplains are available to address your spiritual needs and concerns as well as those of your family members. Our chaplains pray with you and your family as well as provide encouragement and support. They are knowledgeable about various religious traditions and customs and can help facilitate arranging services to meet your specific needs. Chaplains play an integral role in the care of a hospice patient through providing reassurance and spiritual support in the patients’ journey no matter what their affiliation or beliefs may be. Chaplains promote a sense of well-being and help prepare for end of life through spiritual support and guidance. Chaplains are compassionate listeners and can ease the stress of emotions that your family and caregivers may be experiencing.
Our team of medical social workers are also part of the support system and will reach out to identify and support any needs you may have. Our team is expert in numerous, required documents that may include power of attorney forms (durable/medical), FMLA forms, utility assistance forms, and more. They are also available to help assess any services you may need and provide appropriate referrals. Our social workers are also very knowledgeable regarding placement or provider services and will provide support and resources.
Upon the passing of a loved one, the bereavement process begins, and chaplains and social workers continue offering coping practices for the family and caregivers.
